Lifestyle planner for managing changes in nutrition and exercise regimes—one potential aim of which might be slowing the progression of diabetes, for instance. Allows the user to set personal health goals (such as manage blood pressure and blood sugar, manage cholesterol, reduce weight, reduce waist size, and take walks). Charts record and track progress. The app generates action plans and provides guidance in achieving the goals. The affiliate Facebook group allows the user to team up with friends and family, or with other users of the app. Co-developed by US-based patient group, American Diabetes Association (ADA) of Washington State. A Facebook and smartphone app. Requires Facebook registration.
